What is the cost for testing?
The cost for an assessment is a flat fee that varies depending on the scope of the evaluation. This fee is determined at the time of the initial phone call before the testing begins. The fee includes the intake meeting, consultation with schools and other providers, in-person testing sessions, feedback meeting, and a written report. Additional meetings, including attendance at school meetings, are billed hourly. One half of the evaluation fee is due at the time of the intake meeting and the second half is due prior to the parent feedback session.
What forms of payment do you accept?
Checks, Zelle, and cash are accepted.
Do you take insurance?
Dr. Green is not an in-network provider with any insurance companies, but can provide a superbill if your medical insurance includes out-of-network reimbursement for neuropsychological testing. Superbills are provided upon request. It is recommended that you speak with your insurance carrier to determine what is covered, and the specific requirements.
If you would like to check your coverage in advance and procedure codes are requested, some of the codes typically billed are: 90791, 19132, 96133, 96136, 96137, 96146. Diagnosis codes cannot be provided prior to the completion of testing.
What is needed for the initial intake meeting?
Please send any relevant records (e.g. previous testing reports) via mail or email.
What do I tell my child about why he/she is coming for testing?
The answer largely depends on age, developmental level, and functioning of the child. In general, it is advised that parents inform their children that they will be doing some activities that will help us get to know them better. Some are like what they do at school (e.g. reading, writing and math), and we also do things like drawings and puzzles together. Dr. Green is happy to provide more specific guidance to parents during the intake meeting after learning more about the individual child.
Does my child need to prepare for testing?
If your child wears reading glasses, please bring them. A good night of sleep and a meal beforehand are also important. No high sugar meals or caffeine on testing days. Parents are encouraged to pack preferred snacks and/or lunch.
What do parents do during the testing sessions?
Please note my office does not have a waiting room. As such, parents are welcome to roam freely during testing sessions.
